Bridging the continuum of care for heart failure using a community based workforce: A model for the “new” healthcare sector in the management of chronic heart failure (CHF)

      Background: In Australia, specialist CHF programmes enrol only 11% of the CHF population discharged from hospital each year. Within these programmes, only 52% have sustained funding, and the specialist nurses involved have reported working environments with limited resources, infrastructure, and unsustainable workloads.
