Adenosine-Sensitive Ventricular Tachycardia arising from the Middle Cardiac Vein

      We present a rare case of idiopathic adenosine-sensitive VT arising from the epicardial venous system. Early recognition of the typical ECG features of epicardial VT facilitated a focussed approach to mapping and successful ablation from the middle cardiac vein (MCV).
