      In the January issue of Heart, Lung and Circulation, Li et al. highlighted blood transfusion as an important determinant of acute kidney injury (AKI) following total aortic arch replacement, in a retrospective study including 421 patients with acute Stanford type A aortic dissection [

      ]. The index findings are indeed noteworthy from a perioperative management perspective. Nonetheless, the following observations surrounding possible confounding mandate elucidation, given the multifactorial aetiology of post-cardiac surgery AKI.
